POOLER: Mrs. Hazel Smith Forrest, 92, died on Sunday, January 17, 2010 at her daughter's residence surrounded by her family.
She was born in Dublin, GA to the late George W. Smith and Vida Davis. She retired from Union Camp and was a member of Faulkville Baptist Church. She is preceded in death by her husband, Jefferson R. (Jay) Forrest; son, Billy Smith; brother, Troy E. Smith and great-grandson, Matthew Moore.
She is survived by her daughter, Joyce and Dock Baker of Pooler; step-son, Robert and Mary Forrest of Springfield; grandchildren, Debra and Steve Brookins of Haddock, Billy Smith of Gray, Peggy and Bradd Craft of Pooler and Patsy and Bradley Mallard of Pooler, Bobby and Tina Forrest of Springfield and Sarah Morgan of Pooler; great-grandchildren, Andrew Smith, Catherine Kuhn, Chelsea and Savannah Brookins, Beth Smith, Bryce Mallard and Taylor Craft; sisters, Mary Edwards, Mazie McLendon and Carolyn Handley; sisters-in-law, Daisy Toler and Mildred Johnson.
Visitation will be Monday, January 18, 2010 from 6:00 until 8:00 p.m. at the funeral home.
Funeral services will be held on Tuesday, January 19, 2010 at 11:00 a.m. in the funeral home chapel with burial to follow at Hillcrest Abbey West Cemetery on Dean Forest Road.
